Waiver & Release:

By signing the form at the bottom, I acknowledge that I am responsible for dropping off and picking up my child at the times listed as pertaining to this class. It is my responsibility as the child's parent/guardian to furnish safe reliable transportation to and from the class.

Transportation:

Please list all who are authorized to pick up your child in the form field titled "Transportation & Medical Emergencies."

We will need a contact phone number and copy of their ID. We take the safety of your child very seriously and will check the driver's license before releasing your child.

Medical Emergencies:

Please add any information regarding physician contact, allergies, medications, and/or hospital choice in the form field titled "Transportation & Medical Emergencies."

By signing the form at the bottom, the Fly Arts Center, its volunteers, and operators, will seek medical treatment for the participant in the event we are unable to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or conditions, and declare the participant to be in good physical and mental health. If necessary, the parent/guardian authorizes that the child be transported to a nearby hospital.
